Main Responsibilities: Personal Care & Assistance
- Create and Review Care Plans: Develop, monitor, and review tailored care plans that reflect each individual’s needs, preferences, and aspirations, championing our Philosophy of Care.
- Initial Client Assessments: Conduct warm, friendly visits to prospective clients, ensuring a smooth transition into our caring community.
- Record Keeping & Quality Assurance: Maintain clear, accessible records on care and medications to guarantee consistent, high-quality service.
- Involve Loved Ones: Actively include families and advocates in the care process, providing regular updates on well-being and progress.
- Quality Oversight: Supervise care delivery to ensure it embodies our standards and the client’s personal choices.
- Support & Delegate: Guide carers through care planning and observations, fostering a collaborative team environment.
Social & Emotional Support
- Empower Independence: Encourage clients to participate actively in their routines, maintaining social connections and community involvement.
- Emotional Wellbeing: Assist clients and families in understanding health-related behaviors and emotional changes, providing reassurance and support.
- Family Support: Offer guidance to relatives navigating complex emotions and family dynamics related to care decisions.
- Audit & Compliance: Oversee regular audits, ensuring continuous improvement and adherence to standards.
Communication, Monitoring & Reporting
- Proactive Monitoring: Regularly assess and document clients’ wellbeing, adjusting plans swiftly to meet evolving needs.
- Emergency Response: Participate in an on-call rota, ready to respond to urgent situations outside regular hours.
- Accurate Documentation: Keep detailed, confidential records, ensuring safe storage and easy access.
- Health & Safety: Monitor health assessments, liaise with relevant teams, and escalate concerns with professionalism and sensitivity.
- Incident Management: Support investigations and ensure transparency in handling incidents and complaints.
- Community Engagement: Represent our organisation at community events, boost referrals, and forge new connections.
Ensuring Safety, Dignity & Independence
- Work Environment: Promote a safe, healthy workplace for staff and clients by adhering to health and safety regulations.
- Safeguarding: Maintain respectful boundaries, uphold confidentiality, and protect clients’ rights and dignity.
- Staff Supervision: Oversee and mentor care staff, guiding them in daily tasks, ensuring care standards are met, and personal development is supported.
- Training & Development: Conduct performance reviews, facilitate ongoing training, and support career growth for team members.
Leadership & Team Development
- Staff Supervision & Support: Lead by example, provide clear guidance, and support carers in delivering exceptional person-centred care.
- Performance & Recruitment: Participate in interviews, support induction, and manage team performance to maintain high standards.
- Client & Family Feedback: Gather and respond to feedback to continuously improve service quality.

What we require from you?
Essential Education, Qualification & License's
● Level 3 qualification in Health & Social Care (or must have a Level 2 qualification and be willing to start the Level 3 qualification within six months of commencing the role).
● Eden Alternative Training successfully undertaken or willing to do so.
● Evidence of having completed training related to clients with care needs
● Enhanced DBS
● Clean Driving License
Job Type: Full-time
Pay: £37,000.00-£37,300.00 per year
