As our Out of Hours Care Coordinator, you’ll be the heartbeat of community care during evenings and weekends, guaranteeing seamless, high-quality support for our users. You will coordinate care schedules, build strong communication channels with families and carers, and make critical decisions that impact people’s lives — all while working in a passionate, supportive team committed to excellence.
What You’ll Do:
Lead with Compassion & Precision:
- Manage and optimize care schedules and rotas, tailoring services to each individual’s needs and preferences.
- Act as the go-to contact during evenings and weekends, handling emergencies with calm expertise.
- Engage proactively with users, families, and carers, ensuring everyone’s needs are met and expectations are exceeded.
- Work with internal and external health professionals to facilitate smooth home visits and integrated care.
- Organize training, quality assurance visits, and team development opportunities to elevate our service standards.
- Collaborate closely with the Care Team to ensure everyone is supported and informed.
- Participate in interviews and onboarding, helping us find and grow our outstanding care team.
Keep Everything Running Smoothly:
- Monitor attendance and scheduling, ensuring every visit is completed on time.
- Communicate effectively about absences and schedule changes to ensure no one is overlooked.
- Maintain detailed records and documentation that support quality, safety, and compliance.
- Share user feedback, report incidents, and contribute ideas for continuous improvement.
- Identify opportunities for business growth and enhanced service delivery.
Champion Safety, Dignity & Respect:
- Uphold all policies related to safety, confidentiality, and safeguarding.
- Escalate concerns immediately to protect our service users and staff.

What You Bring:
- Experience in care coordination or a similar healthcare role.
- Exceptional communication and empathy skills.
- Calmness under pressure and quick decision-making abilities.
- Strong organizational and problem-solving skills.
- Knowledge of healthcare regulations and best practices.
