Job Title: Out of Hours Care Coordinator
The Out of Hours Care Coordinator is responsible for ensuring the timely delivery of community care to users/clients during evenings and weekends. This role involves monitoring and coordinating care schedules, communicating with users and families, managing carer availability, and liaising with health and care service providers. The Out of Hours Care Coordinator ensures that care is delivered in line with Avante’s Philosophy of Care and adheres to the organization’s Quality Manuals, Policies, Procedures, Guidelines, and Work Instructions.
Main Responsibilities:
- Oversee and manage the preparation and monitoring of the home care service schedule and carers’ rota, ensuring alignment with service users' needs, preferences, and priorities.
- Participate in an on-call rota system for emergencies and out-of-hours care coordination.
- Liaise with users and families to ascertain availability and scheduling needs, and communicate with carers regarding their availability and planned absences.
- Collaborate with the Field Care Supervisor to understand users’ needs and carers’ skills for optimal carer-user matching, including arranging cover for planned and unplanned absences.
- Ensure effective scheduling and templating to optimize timetables for users and carers.
- Organize visits from internal services, such as Quality and People & Organisation Development, for the purposes of training, quality assurance, and support.
- Assist with the planning and implementation of training activities for Field Care Supervisors and Home Carers in conjunction with internal services.
- Coordinate with external healthcare professionals, social workers, and case managers to facilitate home visits and integrate health and care services for users.
- Manage the daily schedule of the Care Team, providing guidance on travel itineraries and addressing issues that might affect timely visits.
- Collect feedback from users and families regarding service delivery and carer punctuality.
- Collaborate with other Care Coordinators to ensure comprehensive care coverage.
- Participate in conducting interviews and helping to onboard new team members.
Social and Emotional Support:
- Provide compassionate support to users and families, actively listening to their concerns and assisting them in managing the emotions associated with receiving personal care.
**Communicating, Monitoring, Documenting, and Reporting:**
- Monitor carer attendance to ensure all scheduled visits are fulfilled daily.
- Communicate planned and unplanned absences to Payroll in a timely manner.
- Maintain clear and accessible records of scheduling, rotas, and absences for all stakeholders.
- Ensure user and family input is gathered systematically and that records are regularly updated.
- Report significant changes or concerns to the line manager and timely relay any scheduling or rota adjustments to colleagues, users, and families.
- Engage with the Home Care Management Team to contribute to the quality improvement of home care services and suggest enhancements for service integration.
- Assist management in investigating incidents and complaints per organization policy.
- Ensure documentation of compliments, incidents, and complaints within the electronic database, responding to complaints thoroughly and notifying relevant bodies as necessary.
- Inform the manager of potential clients or business development opportunities.
Ensuring Safety, Confidentiality, Dignity, and Independence:
- Adhere to all office systems and procedures to maintain operational compliance.
- Immediately escalate any issues that jeopardize the health and safety of service users, families, or staff.
- Ensure all scheduling and rotas comply with regulations and policies related to safety, data protection, employment, human rights, and protection from abuse.
- Identify, address, and report non-compliance concerns, safeguarding the dignity and rights of users and staff.
- Maintain clear boundaries between professional and personal relationships with users, families, and external care providers.
Responsibilities:
- Follow Avante Policies, Procedures, Guidelines, and Work Instructions as outlined in the Residential and Corporate Quality Manuals, and stay current with updates and training.
- Uphold Avante Care and Support’s commitment to Equality, ensuring understanding and application of the Equal Opportunities Policy.
- Attend all mandatory training sessions and CPD activities necessary for the role, informing the manager of any training that has not been provided or attended.
Qualifications and Skills:
Please Note if you are not a commutable distance from either Bexleyheath or Faversham you will not be considered for this position
- Previous experience in care coordination or similar role in the healthcare sector.
- Strong communication and interpersonal skills.
- Ability to work efficiently under pressure and in emergency situations.
- Good organizational and administrative skills.
- Understanding of healthcare regulations and best practices.
